Position Description

Under the direction of the Project Manager, the Community Health Worker is responsible for conducting community organizing, prevention outreach, and education activities for identified priority populations. The Community Health Worker is a trained public health worker and an inspiring leader capable of coaching, mobilizing and motivating community residents, while serving as a bridge between diverse communities, health care and social service systems, and local and state agencies. The Community Health Worker will provide one-on-one and group prevention health education, training, resource connections, and appropriate follow up to enable priority populations to achieve optimum health and wellness. The Community Health Worker will adhere to established procedures for outreach, education, advocacy, and referrals. The focus area for The Community Health Worker will vary according to program goals. The Community Health Worker must become knowledgeable of focus area content, approved messaging and required procedures to advocate for community members.


1. Establish relationships with priority populations and provide general support and encouragement.
2. Conduct intake interviews and assess strengths, needs, and opportunities to improve overall health and well-being of individuals and communities.
3. Serve as a liaison and help connect community members to local programs and services while advocating for community members in the process.
4. Assist community members with enrollment in programs and benefits for which they are eligible.
5. Encourage cultural humility among health and social service providers serving priority populations.
6. Amplify the voices of the community members with local and state agencies to inform programs, resource allocation, and interventions.
7. Build capacity within Fresno BHC and in the community at large to address social determinants of health.
8. Actively participate as a member of the project team to ensure community needs, access to resources and supports are identified and addressed.
9. Assist community members by providing prevention health education, self-development training, and connect to resources to address challenges.
10. Make follow up phone calls, home visits, or visits to other appropriate settings.
11. Provide individual and group training on whole health concepts.
12. Assume lead responsibility for organizing and maintaining project files and databases in electronic and hard copy formats.
13. Serve as the first contact for residents, partners, and outside parties.
14. Ensure that all Fresno BHC partners receive and provide timely communications.
15. Coordinate logistics for meetings, events, conferences, workshops, trainings, and special events.
16. Maintain an organized system of tracking, monitoring, and prioritizing tasks.
17. Participate in assigned local, regional, and state meetings, events, and activities related to Fresno BHC efforts.
18. Travel as needed.
19. Maintain flexible hours as some meetings and program activities may occur outside normal office hours, on weekends, and in various locations.
20. Other duties as assigned.


The Community Health Worker must work well under pressure and gracefully handle stressful situations; will think critically/logically; have follow-through (must be able to follow up on, keep track of, and communicate status of multiple assignments); work independently once given directions; and manage time efficiently and adhere to deadlines.
Must possess a high school diploma/GED and have at least two years of experience working with diverse communities in a similar position, preferably in a non-profit setting.
Must possess a valid California Driver’s License and verifiable automobile insurance and have the use of a reasonably reliable automobile for use on the job, ability to pass background checks and LiveScan fingerprinting and must be able to occasionally lift up to 40 pounds.

11. Demonstrated ability to develop and maintain effective working relationships with co-workers, partners, and people form diverse backgrounds and communities; ability to work independently and as a member of a team.
12. Ability to speak, read, and write a second language (such as Hmong, Spanish, Punjabi or other common language in Fresno County) is highly preferred.

Physical Demands

While performing the duties of this job, the employee is regularly required to use hands and fingers. The employee frequently is required to stand, walk, sit, reach with hands and arms, kneel, talk, and hear. The employee must occasionally lift and/or move up to 40 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.

Work Environment

The position typically works indoors in an air-conditioned office, with a mixture of natural, incandescent, and florescent light. Typical noise levels are muted by acoustic ceilings, carpets, and sound-deadening wall panels. Frequently, the position is called upon to work outside of the office at a public meeting or event. Some of these occur indoors, while others occur outside with exposure to weather and temperature extremes and moderate noise levels. This position requires travel and the ability to interact with others in both small and large group settings. At times, this position may include periods of extended physical activity, such as walking/standing at community events and/or moving and setting up equipment. The employee is frequently required to meet multiple demands from several people.
